Chronic Disease Management including:


COPD /Asthma

Held by: Practice Nurse

The practice nurse provides this service during her surgery sessions.



 For patients who have been diagnosed with hypertension we offer a structured hypertension management service for routine monitoring and this is provided by the nursing team.


Annual Review

You will be offered an appointment with one of the nursing team.  They will perform relevant investigations such as blood pressure reading, blood tests, weight and urine testing if required.

How to get a health checkup

Newly registered patients, those aged 75 or over who have not had a consultation in the past year and adult patients under 75 who have not had a consultation in the past three years all have the right to request a health check consultation with the HCA at the practice, patients aged 45 and over who have not had a blood pressure check in the last 5 years are also encouraged to have this checked. Please call reception to arrange an appointment.

Do you or a Member of your Family need any Advice or Support in relation to their Care and living an independent life at home?

If so you can make an appointment to speak with our Health & Social Care Co-ordinator Angela Spencer who will be at Headcorn Surgery on Wednesday Morning’s

between 9.30 am and 12.30 pm


Some of the Areas covered will be:- aids & adaptations, benefits, befriending, blue badge, care at home, carer support, day care, fire safety checks , gardening, hot meal delivery housing issues, lifeline/keysafe, mobility issues, shopping, telecare  and much more.
